The Organizational Information section of the Project narrative requires us to illustrate how the program meets the statutory requirement of being staffed by families who have expertise in federal and state public and private health care systems … and to include a MOA/U between the organization and these staff. Our project is staffed by parents of CSHCN who are also employees of our organization. Is an MOA between our organization on those employees necessary in that case?Can you explain any more detail on the requirement that project staff includes health professionals?

For organizations straddling a State line, serving families in two states, like Kansas City (Kansas and Missouri), can you give guidance on how to apply?

Question about Baselines: If you are a continuous applicant and you set numbers for Y4 of the current funding cycle but exceeded your numbers, do you use the actual numbers as your Baseline for Y1 for the new cycle or the numbers proposed for Y4?

Does the diversity impact statement and inclusion plan need to be part of the application or is a grant deliverable?

Will additional support be given to organizations to be able to meet the evaluation requirements?

Thank you for taking the question regarding the Tribal Chairman/Governor's office. The tribal health care organization is located in Alaska and is responsible for providing health care to multiple tribes. How could we address the preference for involvement of the tribes we work with?

Will participants attendance/contact information be shared on site?

Does the hiring of family members be immediate or progressive, allowing the program to build the competency of staff, if needed?

The target areas are specific to State or Tribal, etc. In a state as small as WY, would collaborating with tribal health be possible? And is the 10% increase from baseline and annual requirement?

I'm not finding the DGIS link for the data in the NOFO, what section is it in?

We typically upload letters of AGREEMENT (LOAs) as Attachment 4 whereas letters of SUPPORT (LOSs) we upload as Attachment 9-15 and might be from public officials who are not committing service but rather simply stating support of the proposal. Is that acceptable?

There were several questions about who are considered "health professionals" as required members of the team. Can you address who/what experience you consider to meet that criteria?
